Low Testosterone Signs Men Over 40 Should Notice

Testosterone often gets reduced to the image of bulked-up guys curling weights, but the hormone has far more reach. It influences physical strength, sexual function, mood, cognition and overall vitality.

Many men blame low energy, irritability or a disappearing sex drive on stress or age, but these shifts can signal low testosterone, also known as hypogonadism. Spotting it early and getting medical input can make a long-term difference.

How Low Testosterone Shows Up

Ingrid Singels, Associate Director of Pharma Dynamics’ Scientific Division, notes that testosterone supports the traits many men rely on without thinking about it. When levels fall, body and mind both feel the impact.

Reduced libido, erectile difficulties and fewer spontaneous erections are early red flags. Men may notice muscle loss, stubborn fat gain and slower recovery after exercise or injury. Bone density can drop, increasing fracture risk. Mood and mental clarity take a knock too, with fatigue, depression, irritability and poor concentration becoming common. Research also links chronically low testosterone to higher cardiovascular risk and increased mortality.

Why Levels Drop and Who Is Most at Risk

Testosterone naturally declines with age, roughly one percent a year from a man’s thirties onward, but never to zero. Low levels can come from issues in the testes themselves, known as primary hypogonadism. Secondary hypogonadism stems from the brain not sending the signals needed for testosterone production. Contributing factors range from obesity and type 2 diabetes to chronic illness, HIV, certain medications, sleep problems, alcohol use and major stress.

A gradual age-related type, late-onset hypogonadism, is more common in men who are overweight or who live with conditions like cardiovascular disease and type 2 diabetes. Singels points out that symptoms creep in quietly, which is why men often miss the warning signs.

How to Reduce Risk and Catch Problems Early

Not every case can be prevented, but smart habits slow the decline. Keeping a healthy body weight, especially reducing abdominal fat, helps the endocrine system function properly.

Strength training and regular exercise support muscle mass and may help maintain hormone balance. Good sleep, managing chronic health issues, limiting alcohol and eating well all support healthier testosterone levels.

Screening matters too. Pharma Dynamics has digitised the internationally recognised ADAM Screening Questionnaire so men can check their risk online, download their results and share them with a doctor. Singels notes that this helps men start conversations they often avoid.

When to See a Doctor and What Treatment Looks Like

If symptoms and screening results suggest a problem, the next step is a medical evaluation and blood tests. Treatment depends on the cause and the man’s overall health. Doctors often start by tackling underlying issues such as excess weight or high blood sugar, which alone can improve testosterone levels.

For men with confirmed hypogonadism and noticeable symptoms, Testosterone Replacement Therapy may be recommended. It is available as gels, patches, injections and implants.

Long-acting injections offer steady hormone levels for weeks or months. Since testosterone is a regulated medicine in South Africa, treatment must be properly monitored by a doctor.

Why Paying Attention Matters

Low testosterone is not just about sex. It affects strength, mood, long-term health and a man’s overall sense of vitality. Singels stresses that ignoring the signs only delays treatment. Early screening gives men the chance to address the problem before it affects more aspects of their lives.
